It has been brought to our attention that apparently some CPA Ontario members were confused by the consent form included in our application form. We apologize for any confusion and wish to inform CPA Ontario members that we at CPA Professional Liability Insurance Plan are unable to submit your insurance information into CPA Ontario's portal. Submitting this information continues to be YOUR responsibility. If you cannot locate your policy information, please contact a program representative who will send you a certificate of insurance which will assist you with completing the information required for CPA Ontario's portal.
