If you are interested in becoming an AICA member or to receive a free quotation, please complete an AICA application form. Be sure that you fully complete all sections and that you sign and date the form in Section O, then submit your application by fax, mail, or by email attachment, to the attention of the Program Representative for your region. Upon our receipt, the Program Representative will prepare a quotation based on the information you have disclosed on your application. A copy of our quotation will be sent by fax or email and the original by mail.

Please DO NOT complete this application if you are a CA in Quebec.

Note:

  1. By completing an application form, there is no obligation to purchase a professional liability insurance policy from AICA Services Inc. Furthermore, AICA's receipt of a completed application form does not bind it to provide an insurance policy.
  2. The quotation will be based on the accuracy of the information provided. Please read carefully all statements and questions on the application.
  3. A signed copy of the application will be required prior to binding the policy.
  4. To submit an application form, print out and complete the form and send it to us by fax 416-204-3418, or by mail to AICA Services Inc., 277 Wellington Street West, Toronto, Ontario, M5V 3H2, or by email as an attachment to your Program Representative.

Modes of Payment

  1. Lump sum payment by cheque;
  2. Credit card (VISA or MasterCard) payment by phone for premiums of $1,500 and under; or
  3. Monthly payment plan at an interest rate of 10%, using post-dated cheques

AICA's flexible payment plan allows members to reduce their cash outlay at policy inception/renewal by paying their premiums throughout the year in twelve equal instalments. Administered in-house, this convenient plan is offered at an annual interest rate of 10%. The monthly payments are withdrawn on the same day of each month as your policy's effective date.

If you wish to subscribe to the plan, please send a written request with your application form or contact your Program Representative. We would then send you a finance contract for your review, signature, and remittance.

Cheques must be made payable to AICA Services Inc. and remitted by mail.
